To record your participation in the educational sessions, use the Colorado Verification of Attendance Form unless stated differently by your state. State forms are located on the table as you enter the educational session room.

Things to keep in mind to ensure that you receive credit for the approved sessions:

  • Read all documents carefully.
  • You MUST attend the entire session.
  • The Colorado Verification Form MUST be punched at the end of each attended session. STATES DO NOT ALLOW THE FORM TO BE PUNCHED AT ANY OTHER TIME.
  • If your state requires the CPCA Form, please leave the white and yellow copy with the CPCA monitor, UNLESS YOUR STATE REQUIRES YOU TO SEND THE ORIGINAL.
  • If your state requires a course number, you MUST enter it on the Colorado Verification Form OR your specific state's verification form.

Have the CPCA form punched. Attendees keep the WHITE & YELLOW copies of the NPMA form and turn the PINK copy into the CPCA monitor. It is the attendee’s responsibility to submit the white copy of the form and a copy of the meeting agenda at your time of license renewal.
